Pigmented neuropathy (MS) is a pigment-producing neuropathy, a rare variant of neuroblastoma, which was treated in March 2019 and is now reported below.
case 1: patient, female, 14 years old, due to left lower limb pain six months hospital admission, admitted to the hospital: the left side of the spine, the left side of the spine 4, 5 gap pain, and left lower limb radiation pain, double lower limb muscle strength 5 level, the left lower limb straight leg raised test 40 degrees positive, strengthen the test positive.
lumbar magnetic resonance tips: waist 4 to 5 vertebral plane T2WI shuttle low signal shadow, size 4 cm x 1.7 cm, reinforced scanning uniform enhancement (Figure 1).
in the whole hemp downstream vertebral tube occupal, the surgery can be seen in the lesions are black, there is a clear boundary, texture recognition, blood supply rich, can be seen a nerve root into the tumor, the surgery in the whole cut tumor.
postoperative pathology tips: CK (cook, stove, PR), SOX-10 (cook, S-100), HMB45 (plus), MelanA (plus), Ki-67 (5%), NeuN (cooker), diagnosis of melanoma CLASS I (Figure 2).
Case 2: The patient, male, 55, was admitted to hospital in March due to head and neck pain.
admission: the right side of the facial pain temperature slightly reduced, the residual nervous system did not see positive signs.
the right-hand skull MR prompt the right-hand rear cranial nest visible irregular knots, size 3.4 cm x 2.2 cm, the boundary is clear, the signal is uneven, T1, T2 for high and low signal mixed, the edge can be seen T2 low signal ring, enhanced after uneven reinforcement, on-screen ventricular system expansion (Figure 3).
in the whole hemp through the right side of the rock before the road to remove the tumor, the tumor is seen in the surgery is black, texture, blood supply rich, tumor and trigeminal nerve boundary is not clear, in the trigeminal nerve gap growth, the surgery in the whole cut tumor.
postoperative pathology suggests that S-100 (partially suspicious), HMB45 (plus), MelanA (plus), SOX (-), Ki-67 (cells plus), and the diagnosis of melanoma -class 1 (Figure 4) pigmented neuropathy (melaic schwannoma, MS) is a rare type of tumor, first reported by M Illca WG in 1932, as reported in the literature to date.
MS is more common among women, the ratio of men to women is 1:1.4, the peak age of the onset of 30 to 40 years old.
MS is good at the spinal cord after the root, sympathetic nerve chain, cranial nerve, rare in the peripheral nerves and internal nerves.
the 2013 edition of WHO Soft Tissue Tumor Classification, the biological behavior of melanocyte stoma was identified as ICD-1 (locally invasive), defined as a rare late-onset metastatic neuropathy, consisting of a blood-mongocin cell that produces melanin.
tissue credit type is divided into non-gravel body type and gravel body type, gravel body size of about 50% of cases with PPKAR1A mutation Garney syndrome.
pathological characteristics: the vast majority of tumors are rich in cells, tumor cells are shuttle-shaped, multi-angled or epithelial-like, containing a large number of melanin, cells are often round or egg circle.
immune groupification tips: S-100, HMB45, MelanA, Leu-7 are positive, GFAP does not express.
electroscope scans observed that cells have neuroblastoma characteristics, and there are cyanoplasmin small bodies with different stages of development.
previously believed that melanin neuropathy is a benign inert lesions, Torres-Mora and other studies show that MS in situ recurrence rate of 35%, metastasis rate of 44%.
treatment is based on surgical excision, after the full removal of surgery, long-term follow-up recurrence and distant metastasis are required.
radiotherapy is required after surgery for the removal of incomplete persons, with a total dose of 54GY.
cases of recurrent metastasis are insensitive to radiotherapy and chemotherapy.
most doctors do not know enough about the tumor, easy to judge for malignant melanoma caused by surgery to cut incomplete, the rapid freezing in surgery is conducive to the identification of malignant melanoma.
