Adult epilepsy is associated with aggressive atrophy of the cortique, which is more than twice as fast as normal aging.
goal of this study is to determine whether successful epilepsy surgery blocks the thinning of the sexual cortique.
In this longitudinal case-controlled neuroimaging study, the researchers included subjects with pre-temporal lobe excision (n s 29) or after (n s 56) one-sided temporal lobe epilepsy (TLE) and healthy volunteers with a ratio of age and sex (n s 124).
researchers measured the thickness of the cortical layer in an MRI scan of the pairing structure of all participants and compared the gradual thinning between the groups using a linear mixing effect model.
compared to the thinning of the aging-related cortology in healthy subjects, progressive cortitological atrophy was found in a point-by-point analysis prior to TLE surgery, a two-sided lesions located outside the temporal lobes on the same side.
these areas, we observed the left side of the TLE pre-surgery case (left TLE 0.0192±0.0014 with health volunteers 0.0032±0.0013 mm /year, P slt;0.0 001) and right (right TLE 0.0198±0.0016 with health volunteers 0.0037±0.0016 mm / year, P .lt;0001) accelerated thinning.
the left front temporal lobe (0.0074±0.0016 mm / year, P s 0.0006) or right (0.0052±0.0020 mm / year, P s 0.0006) after surgical removal, the cortique of these areas is reduced.
the postoperative TLE group and the preoperative TLE group directly on verteocity analysis, the postoperative thinning area was in two hemispheres, especially but not limited to the preoperatively affected area.
who did not have seizures at all after surgery did not see much thinner conductivity than observed during normal aging.
patients with postoperative seizures continued to accelerate to thin after surgery.
, successful epilepsy surgery can prevent the aggressive cortical atrophy observed in TLE and may have neuropliative effects.
effect is more pronounced in people who still do not have seizures after the temporal lobe is removed, returning the rate of atrophy to normal aging levels.
these results provide evidence for epilepsy surgery to prevent further brain damage and provide incentives for early surgery in responsive TLE.
