Three, security risk research and judgment

(1) Basic requirements

Establish a safety risk research and judgment system, improve the responsibility system, clarify the work responsibilities of the company's main responsible person, responsible person in charge, various functional departments, each workshop (branch), and each team position, and strengthen target management and performance evaluation.

In accordance with the principle of "suspect and risk must be investigated, risk must be judged, and risk must be controlled", establish a safety risk research and judgment work process covering all employees and the whole process of the enterprise.

3.
While carrying out daily team handovers, workshop production scheduling meetings, and plant-level production scheduling meetings to arrange production tasks, it is necessary to simultaneously study and judge the safety risks of various tasks and implement safety risk control measures.

(2) Key content

1.
The safe operating state of the production plant.
Whether the main process parameters such as temperature, pressure, composition, liquid level and flow rate of the production equipment are within the index range; whether special equipment such as pressure vessels and pressure pipelines are in safe operation; whether the static and dynamic seals of various equipment and facilities are intact and leak-free; Check whether various safety facilities such as over-limit alarm, emergency cut-off, interlocking, etc.
are intact and operate reliably.

2.
The safe operation status of major hazard sources such as hazardous chemical tank farms and warehouses.
Whether the storage tanks, pipelines, pumps, valves, and instrument systems are intact and leak-free; whether the liquid level, temperature, and pressure of the storage tank are operating beyond limits; whether the floating plate may fall to the bottom during the operation of the internal floating roof storage tank; the oil and gas tank area is manually cut Check whether personnel are on duty when water, tank cutting, loading and unloading trucks; whether combustible and toxic gas alarms and interlocks are in reliable operation.
Whether the warehouse stores hazardous chemicals according to national standards and divisions, whether they are stored in excess quantities and varieties, and whether mutually prohibited substances are mixed and stored.

The safety risk of high-risk production activities and operations can be controlled.
Whether the device starts and stops has a plan for starting and stopping, whether the trial production plan has been formulated and verified by experts ; whether various special operations, inspection and maintenance operations, and contractor operations are sound and relevant management systems, whether safety risk identification is carried out in the operation process, and strict Procedure confirmation and work permit approval, strengthening on-site supervision, whether the hot work in hazardous chemical tank farms are upgraded management, etc.

Security Risk Report and Commitment

When assigning tasks for safety risk research and management and control, it is necessary to hand over tasks, tasks, and goals to subordinates, as well as hand over ideas, methods, and safety requirements simultaneously.

For the lower-level security risk reports and commitments, the higher-level organizations should organize forces to conduct assessments to ensure that various security risk prevention and control measures are in place.

The main person in charge shall comprehensively grasp the various work conditions of safety production in accordance with the actual situation of the enterprise, and personally dispatch to ensure that the safety risks of production and operation activities are in a controllable state.

5.
Under the premise of safe operation of production equipment, tank farms and warehouses, controllable risks of high-risk production activities and operations, and implementation of control measures for major hidden dangers, the premise that major safety risks such as special operations, inspection and maintenance operations, and contractor operations are controllable Next, in the name of the chairman or general manager of the company and other main responsible persons, sign the safety commitment every day, announce it outside the main gate of the factory, and upload it to the website of the local safety supervision department.
When the chairman or general manager of an enterprise goes out, he shall entrust a person in charge of the enterprise to perform the work of safety commitments on his behalf.

Security Commitment Announcement

(1) Main content

1.
Enterprise status: It mainly announces the production and operation status of the enterprise on the day and the main activities that may cause safety risks.
If there are several sets of production equipment, some of them are in operation, and some are out of production; whether there are special operations, types, and times in the plant; whether there are maintenance and contractor operations; whether they are in the stage of start-up and shutdown, trial production, etc.

2.
Enterprise safety commitment: On the basis of comprehensive safety risk research and judgment, the enterprise implements relevant safety risk management and control measures.
The main person in charge of the enterprise promises whether all equipment and tank farms are in safe operation on that day and whether safety risks are effectively controlled.

(2) Announcement method

1.
Announcement time: updated at 10 am every day, until 10 am the next day.

2.
Announcement location: the website of the territorial safety supervision department; a display screen set up in a prominent position at the door of the business owner (see the attachment for an example of a safety commitment bulletin board).
The display screen installed by the enterprise requires clear text and image display, and the installation location complies with fire and explosion protection regulations to ensure the safe passage of personnel and vehicles.

(3) Basic conditions.

An enterprise shall not publish a safety commitment announcement to the public under any of the following circumstances:

1.
Failure to establish a comprehensive management system for safety risk assessment and commitment announcements, and related responsibilities are not implemented at all levels;

2.
No control measures have been formulated for major hidden dangers;

3.
If the special operation management measures such as hot fire do not meet the requirements of relevant standards, the safety risk assessment and effective control measures are not taken on the key installations, tank farms, and special operations such as hot fire on the same day;

4.
There is no person in charge of the enterprise on duty during the special period.

6.
Safety risk assessment and commitment supervision

1.
Safety supervision departments at all levels should supervise and guide enterprises to establish a safety risk research and judgment and commitment announcement system before November 30, 2018, and include the company’s performance of safety risk research and judgment and commitments into the content of supervision and inspection.
Enterprises that falsely publish safety commitment announcements shall conduct interviews, notifications, and public exposures, and they shall be included in key supervision targets.

The safety supervision departments at all levels shall conduct statistical analysis on the company's safety risk research and commitment announcements, implement dynamic supervision, and use the company's performance of safety risk research and commitments as an important basis for joint incentives for safe production and trustworthiness and joint punishment for untrustworthiness, and supervise and guide Enterprises consciously implement the main responsibility for safe production.

3.
Safety supervision departments at all levels should strengthen the establishment of a social supervision mechanism, and encourage employees and the public to actively report to the local safety supervision department when they find that the company does not publish or falsely publish safety commitment announcements.

4.
Encourage qualified regions to establish information platforms, based on the inherent safety risk classification of enterprises in accordance with the requirements of the "Guidelines for Safety Risk Assessment and Diagnosis and Classification of Hazardous Chemicals Production and Storage Enterprises (Trial)", and fully integrate the dynamic risks announced by the enterprises under their commitments , Establish a daily corporate red, orange, yellow, and blue security risk classification, and implement classification and hierarchical supervision according to the classification results, and implement log management.

5.
Relevant central enterprises should supervise and guide branches and subsidiaries to establish safety risk assessment and commitment announcement systems, and improve the responsibility system; formulate overall safety risk prevention measures at the group company level, and guide branches and subsidiaries to implement safety in accordance with actual conditions Risk control measures.

All provincial safety supervision bureaus are requested to promptly convey the spirit of this notice to safety supervision departments and related enterprises at all levels in their jurisdictions.
