Introduction The mortality rate of patients with sepsis is high, and it is particularly important to find biomarkers that can help early diagnosis and predict prognosis of sepsis
    .

Studies have shown that heparin-binding protein (HBP) is closely related to organ dysfunction caused by infection
    .

The value of HBP in sepsis has not been reported in the literature
    .

    Recently, the journal Shock (IF=3.
    454) reported a prospective study to explore the role of HBP in the early diagnosis and prognosis prediction of sepsis
    .

Study Overview The study was a prospective, non-interventional, multicenter clinical study
    .

Adult patients who met the following criteria: (1) core body temperature >38°C or <36°C; (2) heart rate >90 bpm; (3) respiratory rate >20/min and (4) self-reported fever/ Chills
    .

HBP and procalcitonin (PCT) were detected in all patients within the first hour after admission
    .

The primary end point was the diagnostic value of HBP for sepsis within the first 72 hours after emergency room admission
    .

A secondary end point was the prognostic power of HBP for early death, defined as death from any cause recorded in the first 72 hours after emergency room admission; another secondary end point was 28-day mortality
    .

RESULTS: A total of 400 patients were admitted to the emergency room, of which 371 had HBP and PCT test results
    .

Results: ➤ HBP was significantly elevated in sepsis patients and positively correlated with PCT and C-reactive protein, absolute neutrophil and monocyte counts, creatinine, bilirubin, and lactate
    .

➤ The sensitivity, specificity, positive predictive value and negative predictive value of HBP greater than 19.
8 ng/ml in the diagnosis of sepsis were 66.
    3%, 44.
    9%, 49.
    3% and 62.
    2%, respectively
    .

➤ The sensitivity, specificity, positive predictive value, and negative predictive value of HBP greater than 19.
    8 ng/ml in predicting 72-hour mortality were 100%, 41.
    0%, 4.
    5%, and 100%, respectively
    .

➤ HBP and PCT alone did not predict 28-day mortality; when used in combination, the sensitivity, specificity, positive predictive value, and negative predictive value were 44.
    8%, 81.
    8%, 17.
    3%, and 94.
    6%, respectively
    .

CONCLUSIONS: HBP can be used as a predictive tool for early diagnosis and early mortality risk in sepsis
    .

However, HBP may change over time, especially in patients with septic shock who require vasopressors
