Some scholars believe that this may be related to the level of folic acid in serum of RA patients.the study was published in "JAMA netw open". Folic acid is a water-soluble vitamin, also known as butterfly glutamic acid.it can be used as a cofactor to promote homocysteine metabolism.elevated serum homocysteine levels can lead to cardiovascular disease.the researchers suggest that the increase in homocysteine levels in RA patients may be due to the consumption of folic acid by commonly used RA drugs, such as methotrexate.683 RA patients were included in this study, and they were divided into three groups according to the folate level in serum: folate level in one group was lower than 4.3ng/ml; folate level in two groups was between 4.3-8.2ng/ml; folate level in three groups was higher than 8.2ng/ml.during the follow-up period of 17 years, 258 patients died of cardiovascular disease.the researchers reported that serum folate levels below 4.3 ng / ml were associated with a 50% increased risk of cardiovascular death.after adjusting for demographic characteristics, body mass index, smoking history, RA drug use, and comorbidities, the researchers found that patients with folate levels of 4.3-8.2 ng / ml had a 48% lower risk of cardiovascular death, and patients with folic acid levels above 8.2 ng / ml had a 56% lower risk of cardiovascular death.that is to say, when the serum folic acid level of RA patients is not lower than 4.3 ng / ml, the risk of cardiovascular death will be reduced.the lead author of the study, kalyani sonawane, said: "our study is the first to demonstrate the relationship between serum folic acid levels and cardiovascular death risk in RA patients.it is suggested that RA patients taking anti rheumatic drugs should know more about the drug-related risks and take corresponding measures."2 what is the safety and efficacy of metformin in systemic lupus erythematosus? Scholars from three hospitals in Shanghai studied this.the study was published in the lancet rheumatology, a multicenter, randomized, double-blind, placebo-controlled trial.140 adult SLE patients without diabetes who had at least one lupus activity and received prednisone (≥ 20 mg daily) for the previous 12 months were included.patients were randomly divided into metformin group (67 cases) and placebo group (73 cases). The main end point of the study was the patient's disease activity index (selena-sledai) at 12 months.after 12 months of follow-up, there was no significant difference in lupus activity between the two groups. There were 14 patients (21%) in the metformin group and 25 (34%) in the placebo group (HR 0.68, 95% CI 0.42 – 1.04, P = 0.078). compared with the placebo group, patients receiving metformin had a higher incidence of gastrointestinal adverse events, and the incidence of non disease active serious adverse events was similar between the two groups. the incidence of infection events in metformin group was significantly lower than that in placebo group (25% vs. 44%, P = 0.022). no patient died due to treatment in both groups. although the trial was not sufficient to demonstrate the efficacy of metformin in the treatment of SLE, the data provided the basis for future large-scale trials. what is the correlation between 3mri and the prognosis of RA patients? A recent study has shown that there is a correlation between magnetic resonance imaging (MRI) of hand joint damage in RA patients and patients' reported loss of function during continuous clinical remission. the study was published in the International Journal of rheumatic diseases. The researchers conducted wrist / hand MRI and wrist / hand / foot X-ray examination in 114 RA patients with clinical remission. After that, patients gradually reduced the use of anti rheumatic drugs, so as to explore the relationship between the damage reflected by imaging examination and the prognosis and health status of patients.
