    BACKGROUND:hepatitis B core-related antigen (HBcrAg) is a new type of serum virus markerRecent studies have shown that their levels are associated with the risk of hepatocellular carcinoma (HCC) in patients with chronic hepatitis B (CHB)The purpose of this study was to assess the accuracy of serum HBsAg and HBcrAg levels at baseline to predict HCCmethod: since December 2005, 1,400 PATIENTs with CHB have been treated with nucleotide (t) analogues (NA) inRetrieved serum samples they stored at the baseline to measure HBsAg and HBcrAg levelsThe main endpoint is the cumulative incidence of liver cancerresults: 85 (6.1%) patients inhad HCC events within an average of 45 to 20 months of follow-up periodThrough multivariate analysis, serum HBcrAg levels above 2.9 log10 U/ mL at baseline were independent factors of HCC in patients with HBeAg e antigen (HBeAg) negative (adjusted risk ratio 2.13, 95% CI 1.10-4.14, P -0.025)HBcrAg was stratified in HBeAg-negative patients with a high PAGE-B score (Kaplan-Meier analysis P -0.024) and patients with cirrhosis (P -0.08) with a higher PAGE-B scoreSerum HBsAg was not averagely associated with liver cancer risk in all patients or any subgroupConclusion:serum HBcrAg levels can accurately predict HCC risk in Patients with HBeAg negative CHB in NA therapy
