Background and objective: Recurrence of primary bile ductitis (PBC) after liver transplantation (LT) is commonBear deoxycholic acid (UDCA) is the standard treatment for PBCThe purpose of this study was to explore the effects of preventive use of UDCA on the incidence and long-term consequences of post-LT BBC recurrenceMethod: This retrospective cohort study included 780 PBC transplantpatients in 16 centers and 9 countries between 1983 and 2017 with a follow-up period of 11 yearsOf these, 190 cases were treated preventively with UDCA (10-15mg/kg/d)The main result was histological confirmation of PBC recurrenceThe secondary outcome was the failure of liver transplantation, liver-related death and total cause of deathThe association between preventive UDCA and outcome seiscasts using the multivariately adjusted Cox model and the Limited Average Lifetime (RMST) modelresults: Although the recurrence of PBC significantly reduced the success rate of liver transplantation and the survival of patients, the risk of preventive exposure to UDCA and PBC recurrence decreased (adjusted risk ratio, 0.41; 95% CI, 0.28 - 0.61; p 0.0001), liver-related deaths (0.46; 0.22-0.98; p.05) and total death (0.69; 0.49-0.96; p.05)RMST analysis showed consistent results of 2.26 years (95% CI 1.28 - 3.25) over a 20-year lifetimeExposure to cyclosporine instead of tacmos increases The preventive effect of UDCA on PBC recurrence and total deathconclusion: Preventive UDCA use after liver transplantation in PBC patients was associated with a recurrence of the disease and a reduced risk of deathTreatment regimens combined with cyclosporine and prophylactic UDCA have the lowest risk of recurrence and death of PBC
