J Otolaryngol Head Neck Surg: Preoperative immunonutrition helps reduce complications after recurrent head and neck squamous cell carcinoma patients

Because radiotherapy is not conducive to wound healing, patients with recurrent head and neck squamous cell carcinoma who underwent rescue surgery are at high risk of postoperative complicationsMalnutrition is an additional risk factor, and we assume that preoperativeimmunizationnutrients reduce severity in this high-risk populationstudy included patients who underwent continuous recurrent head and neck squamous cell carcinomaThe results before and after the implementation of preoperative immunonutrition were compared, and the regression analysis and correction of sex, age, body mass index, nutritional riskscreening(NRS 2002), tobacco and alcohol consumption, tumor location, tumor stage and surgical type were analyzed and correctedThe main endpoint was the total complications of surgery within 30 days of follow-upresults, the study included 96 patients (intervention group: 51, control group: 45)Preoperative use of immunonutrition was associated with a significant reduction in total postoperative complications (35% vs58%), and a fully corrected OR was 0.30 (95% CI 0.10-0.91, p -0.034)There was also a significant reduction in length of stay (17 days vs.6 days, p - 0.001)No difference was found between mortality and readmissionThese results are still true in multivariate analysis, the results show that in patients who perform rescue surgery on recurrent head and neck squamous cell carcinoma, preoperative immunonutrition is beneficial to reduce postoperative complications and thus shorten hospital stay By improving tissue regeneration and immune response, immune nutrition may help improve surgical outcomes in this high-risk population
