The study prospectively recruited patients with uncontrolled blood pressure (≥140/90 mmHg) despite the use of more than 3 antihypertensive drugs (including diuretics)

Participants started taking 25 mg/d of spironolactone and increased to 50 mg/d at 4 weeks, while stopping other antihypertensive drugs to maintain a constant average blood pressure
Spironolactone can improve the aortic characteristics of patients with refractory hypertension independently of BP, which may support the hypothesis that aldosterone affects the arterial wall
