Stroke is the most common cause of new-onset epilepsy in adults
    .

The association of poststroke epilepsy (PSE) with mortality varied across studies, but the hazard ratio (HR) for death appeared to increase at the population level

Stroke is the most common cause of new-onset epilepsy in adults

There are no real population-based data on the relationship between ASMs and mortality in PSE patients

In February 2022, David Larsson et al.
from Sweden published their findings in JAMA Neurology , with the aim of investigating whether mortality in PSE patients varies by specific ASM
    .

    David Larsson JAMA Neurology

The cohort study used individual-level data on all patients with acute stroke in Sweden between July 1, 2005, and December 31, 2010, and those with seizures before December 31, 2014
    .

A total of 2577 patients receiving continuous ASM monotherapy were eligible for the study

A total of 2577 patients (1400 men [54%; median age 78 years [IQR, 69-85 years]) were included in the study
    .

Compared with carbamazepine, the adjusted hazard ratio (HR) for all-cause mortality was 0.

The authors concluded that the results of this cohort study demonstrate differences in survival in post-stroke epilepsy patients treated with different ASMs
    .

Patients who received lamotrigine monotherapy had significantly lower mortality compared with patients who received carbamazepine

In contrast, patients taking valproic acid had a higher risk of cardiovascular disease and all-cause death
