Background: Diabetes is the leading cause of chronic kidney disease and end-stage renal disease worldwide
The importance of early detection, risk stratification, and timely management of DKD requires a more comprehensive implementation strategy
OBJECTIVE: To evaluate the effect of the combined Asian Diabetes Assessment (JADE) portal, nurse reminders and team care on multiple risk factors in patients with DKD
Design, settings, and participants: This 12-month multinational, open-label, randomized clinical trial was conducted at 13 hospital diabetes centers in 8 Asian countries from June 27, 2014 to February 19, 2019
Main outcomes and measures: The primary outcome was the proportion of patients achieving multiple treatment goals (defined as ≥3 of 5 goals: HbA1c level < 7.
RESULTS: A total of 2393 patients (mean [SD] age, 67.

Conclusions: This trial found that 12 months of technology-assisted team care improved multiple treatment goals
