    The diagnosis of Green-Barre syndrome (GBS) depends on clinical and electrophysiological indicators and cerebrospinal fluid cytological isolation, but patients vary widely in their performance, clinical course and prognosis.
    with the introduction of ultra-sensitive technologies such as single-molecule arrays (SiMoA), the neural wire light chain (NfL) is becoming the most important biomarker of axon damage in neurology, showing elevated levels of NfH in cerebrospinal fluid.
    studied serum nerve wire light chain (sNfL) levels as a biological marker for the prognosis of Green-Bally syndrome (GBS).
    single-molecule array (SiMoA) was used to measure NfL in the serum (98 cases) and cerebrospinal fluid (CSF) of GBS patients in the Spanish International GBS Results Study (IGOS) and compared it with 53 health control groups (HCs).
    the relationship between the sNfL level and the 1-year functional results was analyzed by the authors.
    Serum NfL levels were higher than HC (55.49 pg/mL vs 9.83 pg/mL, p-lt;0.0001) and cerebrospinal fluid levels were higher than HC (1308.5 pg/mL vs 440.24 pg/mL, p=0.034).
    patients with a history of diarrhoeal diseases were sNfL higher than those with respiratory symptoms or no history of infection (134.90 pg/mL vs 47.86 pg/mL vs 38.02 pg/mL, p=0.016).
    sNfL levels were associated with Guillain-Barr's syndrome disability score and the Comprehensive Disability Scale (I-RODS).
    sNfL levels were higher in patients with simple motion variants and Miller-Fisher syndrome than in patients with sensory motor GBS (162.18 pg/mL vs. 95.50 pg/mL vs 38.02 pg/mL, p=0.025).
    sNfL levels in patients with acute motor axon neuropathy were higher than in other variants (190.55 pg/mL vs 46.79 pg/mL, p=0.013).
    sNfL returns to normal levels after 1 year.
    critical point for highly specific clinically relevant prognosmations: inability to walk independently, inability to run( sgt;248 pg/mL), and ability to run ( slt; 34 pg/mL).
    baseline sNfL levels in GBS patients, the severity of the disease was related to axis mutations, and had independent prognostic value for GBS patients.
