The direct cause of the accident is: due to the operation of hydrolytic reactor process heating up too fast, resulting in overpressure in the kettle, the hydrolytic kettle cover and the lower part of the kettle flante connection tear, containing methanol and other materials flammable and explosive gas burst out of the explosion and caused a fire.
June 15, four people were killed in a fire at Sinopec's Shijiazhuang refinery branch. The direct cause of the accident is: the operator at the top of the chimney anti-corrosion welding operation process, welding slag behind caused polypropylene material mist remover filler fire, high temperature flue gas along the chimney discharge, the operator died of asphyxiation due to high temperature and poisoning.

The above-mentioned accidents have exposed some enterprises' weak foundation for safe production, disregarding laws and regulations on safe production, illegally organizing production without fulfilling administrative licensing procedures, failure to implement the main responsibility for safe production, and lack of safety management.
The General Administration of State Security requires: (1) a deep understanding of the importance and urgency of containing large or more accidents, closely integrating local realities, strengthening the awareness of security risks, and focusing on building a dual preventive working mechanism for risk detection and risk management.
