Recently, researchers have investigated the role of urine spermine and spermine risk scores in predicting high-grade prostate cancer (HGPCa, ISUP grade group ≥ 2).

The researchers prospectively recruited 905 consecutive men with elevated PSA from two hospitals.

    30.
    8% (185/600) and 17.
2% (103/600) of men in this cohort were diagnosed with PCa and HGPCa, respectively, and were significantly associated with lower urine spermine levels.

Between the lowest and highest quartile of spermine results, the risk of PCa increased by a factor of 3 (49.
    3% vs.
    16.
7%), and the risk of PCa in the ISUP grade group ≥2 increased by 3.
    5 times (31.
    3% vs.
    8.
    7%) .

Multivariate analysis showed that PSA, prostate volume (PV), digital rectal examination (DRE) and spermine are independent predictors of PCa and HGPCa.
The spermine risk score and the above factors can achieve the highest AUC, PCa and HGPCa respectively 0.
    78 and 0.
    82.

The sensitivity to HGPCa is 90%, which can avoid 36.
    7% of biopsy and 24.
4% of ISUP level 1 diagnosis, and the negative predictive value is 95.
    4%.

DCA shows that the spermine risk score has a net clinical benefit.

Internal verification and bootstrapping show good recognition and calibration.

Finally, the researchers pointed out that urine spermine and spermine risk scores can identify men at higher risk of HGPCa and reduce unnecessary biopsies.
