World's first: Gene therapy becomes reality; foundational research on hereditary kidney disease also achieves major progress!

A few days ago, a medical news story stunned the world: An infant suffering from an extremely rare genetic disorder—carbamoyl phosphate synthetase I (CPS1) deficiency—was rescued at the brink of death by receiving the world’s first fully personalized gene therapy, custom-designed exclusively for him. The research team used CRISPR gene-editing technology to inject the gene-editing machinery directly into his body, enabling it to identify and correct the faulty gene, thereby restoring the correct DNA sequence. Ten months after birth, he received his third dose—and his condition is now stable. This infant has consequently been named one of *Nature* magazine’s “Top Ten Scientists of 2025.” We cannot help but ask: Can this seemingly science-fiction-like technology be applied to treat hereditary kidney disease? Scientists are actively pursuing this goal! Recently, an animal-based foundational study published in the top-tier medical journal *Nature Communications* revealed the therapeutic potential of gene therapy for autosomal dominant polycystic kidney disease (ADPKD)—the most common hereditary kidney disease. This research team also employed CRISPR gene-editing technology, capable of precisely modifying individual DNA bases without cutting the DNA strand. Researchers administered the gene-editing therapy to a mouse model mimicking human polycystic kidney disease, injecting the editor on postnatal day 14 and day 28.

How can AAV targeting efficiency for the kidneys be improved?

Kidney disease has become one of the leading threats to human health, affecting approximately 13% of the global population. Among chronic kidney diseases, those caused by genetic mutations have an incidence as high as 1 in 1,000; mutations in nearly 80 different genes have been linked to various hereditary kidney disorders. However, due to the structural and functional complexity of the kidney, research and development of gene therapies targeting kidney diseases have lagged behind those for the liver, neuromuscular system, and eyes. Since the late 1990s, researchers have been exploring adeno-associated virus (AAV)-mediated gene delivery to the kidney—both *in vitro* and *in vivo*—and have achieved some success. Yet actual transduction efficiency varies substantially, likely attributable to differences in delivery routes and AAV serotypes. Achieving—or significantly enhancing—exogenous gene transduction in the kidney thus remains a critical bottleneck hindering the advancement of kidney-directed gene therapy. 1. Selection of Administration Route The choice of administration route is a key determinant of gene therapy efficacy. The mammalian kidney possesses stringent filtration capabilities, excluding proteins larger than 50 kDa. Moreover, podocytes within the glomerulus form slit diaphragms with pores only ~10 nm in diameter. Consequently, systemic (intravenous) delivery of conventional AAV vectors rarely achieves sufficient transgene expression in the kidney—even high-dose administration tends to concentrate AAV particles in extrarenal organs such as the liver. To enhance renal transduction while minimizing off-target transduction in non-renal tissues, scientists have recently developed several localized kidney delivery methods beyond systemic administration—including renal artery injection, renal vein injection, intraparenchymal injection, retrograde ureteral infusion, and intraparenchymal–renal pelvis injection (Figure 2). As summarized in the table below, these surgery-based AAV delivery approaches yield distinct spatial patterns of transgene expression in the kidney and possess unique advantages and limitations. Renal vein injection: ① Anesthetize C57BL/6 mice (4–6 weeks old, 15–20 g) and position them supine on the surgical platform; ② Depilate the left abdominal region and make a left abdominal incision to expose the left kidney and renal pedicle; carefully isolate the left renal vein from the pedicle; ③ Clamp the distal end of the renal vein using a micro-hemostatic clip to prevent leakage of the viral solution out of the kidney; ④ Insert a 30G needle into the proximal segment of the left renal vein and inject 50 µL of solution (containing either 5 × 10¹⁰ viral genome copies or PBS); ⑤ After 5 minutes, withdraw the needle, remove the micro-hemostatic clip, apply gentle pressure to achieve hemostasis, then close the incision in two layers.
