    Type A Botulinum toxin (BTX-A) is an option for the treatment of surface muscle spasms (HFS), which has a short-term effect of about 3-4 months.
    active muscle exercise immediately after injection may enhance its effectiveness.
    a small randomized controlled trial (RCT) conducted in a joint study of facial exercises with abnormal facial movements and BTX-A reported no significant reduction in the severity and frequency of facial muscle spasms, but only four HFS patients were included.

    recruited participants of any severity of HFS aged 18-80 who received regular BTX-A treatment every 16 weeks.
    exclusion criteria are stroke, eyelid spasms, dementia, blindness, body dependence, taking anticoagulants and not being able to perform facial exercises under video guidance.

    facial movements include 7 minutes of active and passive facial muscle compression, which is performed immediately under the guidance of the video after injection of BTX-A.

    results: 42 participants were screened.
    completed the study programme with 38 participants.
    analysis of HFS-30 scores showed an average difference of -2.56 points (95% CI -5.47 to 0.35) and -1.79 days (95% CI -4.20 to 0.626), respectively.
    sensitivity analysis, the data showed an average difference of -3.52 points (95% CI -7.04 to 0.001) on HFS-30 scores.
    treatment difference for peak effects was -1.98 days (95% CI -4.88 to 0.92) and duration was 0.18 weeks (95% CI -0.65 to 1.01).
    significant differences in adverse events between the treatment groups, and no participants who withdrew from facial exercise due to adverse events and the combination of BTX-A injections showed meaningful help in adult HFS patients.
