    Cognitive impairment is a common disabling feature in multiple sclerosis (MS), but there is still a lack of precise description of the cognitive phenotype of MS patients.

    This study aims to use a data-driven approach to define homogenous cognitive phenotypes in a large number of MS patients and evaluate their unique clinical and MRI characteristics.

     Research Introduction The researchers conducted cognitive assessments of 1,212 MS patients and 196 healthy controls (HC) from 8 MS centers in Italy, including Rao's concise and repeatable psychological test and the Stroop character color test.

    The researchers defined the clinical and MRI characteristics of each cognitive phenotype through linear regression and mixed-effects models.

     Main findings: ➤The study identified five cognitive phenotypes: "retained cognition" (19%), "mild verbal memory/semantic fluency" (30%), "mild multi-domain" (19%) , "Heavy Attention/Enforcement" (14%) and "Seriously Multi-domain" (18%).

     ➤Compared with other cognitive phenotypes, patients in the "preserved cognition" group and the "mild verbal memory/semantic fluency" group were younger and had a shorter course of disease.

    Physical disability was the least in the "reserved cognition" group of patients, and the most in the "severe multi-domain" group.

    Severe cognitive phenotypes are common in progressive patients.

     ➤In the MRI evaluation, compared with the "preserved cognition" group, the hippocampus volume of patients in the "mild verbal memory/semantic fluency" group was reduced, and the cortical gray matter volume of patients in the "mild multi-domain" group was reduced; The volume of T2-hypersensitivity lesions increased in patients in the /execution group and extensive brain damage was found in patients in the "severe multi-domain" group.

     Research conclusions This study can pave the way for future neuroanatomical research of MS-related cognitive impairment by defining homogenous and clinically meaningful cognitive phenotypes, and help to develop treatment strategies for patients with different cognitive phenotypes.
