Content Introduction Chinese Abstract: Objective: To explore the changes in renal function of patients with diabetes and/or hypertension after transcatheter aortic valve replacement, influencing factors and their influence on prognosis.
Innovation: Explore changes in renal function and influencing factors in patients with diabetes and/or hypertension after transcatheter aortic valve replacement, which provides clinical management basis for changes in renal function in some patients.
Methods: 242 patients with severe aortic valve stenosis who underwent transcatheter aortic valve replacement surgery with diabetes and/or hypertension were enrolled, and were divided into 3 groups based on the estimated glomerular filtration rate (eGFR) before discharge and the percentage change of baseline eGFR: Renal function recovery group, renal function unchanged group, and renal function deterioration group.
Compare the clinical baseline data and prognostic data between the 3 groups, and explore the factors affecting the changes in renal function after surgery.
Conclusion: The results showed that the unchanged renal function group had a lower STS score and fewer peripheral vascular disease complications.
The renal function improvement group had lower baseline eGFR values and lower left ventricular ejection fraction (LVEF).
The mortality rate during hospitalization and 30-day follow-up was higher in the worsening renal function group.
Independent predictors of renal function changes include preoperative STS, LVEF, baseline eGFR, and unexpected incidence of cardiopulmonary bypass.
Among patients with diabetes and/or hypertension, nearly 40% of patients have improved renal function after transcatheter aortic valve replacement.
The deterioration of renal function heralds a worse clinical outcome.
