Exogenetic testosterone therapy (TTh) is currently the most widely used treatment in adult patients with glandular decompression disorders because it increases serum testosterone (T) levels.

T4DM was a randomized, double-blind, placebo-controlled, two-year Phase 3b trial completed at six level 3 medical centres in Australia.
included more than 1,000 people aged 50-74 years, 95cm or more waist circumference, serum testosterone concentration 14-0nmol/L or less, but no pathological gland function reduction, impaired glucose tolerance (oral glucose resistance) A man with a volume test of 2-h glucose 7-8-11-0mmol/L) or a newly diagnosed T2DM (provided OGTT 2-h glucose ≤15-0mmol/L).
participants were given lifestyle interventions and randomly assigned 1:1 to receive intramuscular injections of testosterone (1000 mg) or placebo.
after two years of treatment, 87 (21%) of the 413 subjects in the placebo group developed diabetes, while only 55 (12%) of the 443 subjects in the testosterone group developed diabetes.
groups lost an average of 3 to 4 kg of weight.
results were followed up 2 years later, the placebo group had 87/413 (21%) and 55/443 (12%) participants with 2-h OGTT at 11-1 mmol/L or higher (relative risk 0.59).
, the average change in placebo group 2-h OGTT was -0.95 mmol/L and the testosterone group was -1.70 mmol/L compared to the baseline.
, the therapeutic effect was independent of baseline serum testosterone.
In addition, compared to the placebo group, patients who took testosterone after two years had lower peretone blood sugar, significantly reduced body fat, increased skeletal muscle mass and grip strength, and no difference in quality of life.
results suggest that testosterone has some additional benefits for preventing or reversing newly diagnosed T2DM, in addition to achieving moderate weight loss through healthy eating and increased activity.
Of course, given that men at risk of T2DM are often overweight or at risk of other chronic diseases, the researchers note that weight loss through a healthy lifestyle remains the primary measure.
