▎Editor of WuXi AppTec content team China is a country with a high incidence of gastrointestinal diseases.
High-incidence types include gastritis, peptic ulcer and gastric cancer.
Therefore, early screening of high-risk groups is particularly important for improving the diagnosis and treatment rate
    .

Traditional gastroscopy is the gold standard for diagnosis and treatment of gastric diseases
    .

However, patients with relatively contraindications such as severe cardiac or respiratory dysfunction may be unwilling or unable to tolerate traditional oral gastroscopy
    .

The emergence of a new type of magnetically controlled capsule endoscope has brought new options for the examination of gastrointestinal diseases.
Previous studies have initially confirmed that it has high diagnostic accuracy for gastric diseases, and is minimally invasive, comfortable, safe, without anesthesia, and without cross-infection.
    Risks and other advantages
    .

However, the use of magnetically controlled capsules for gastric examination is still in the early stages of clinical application
    .

At the same time, the magnetic control capsule endoscope technology has rapidly developed to the third generation: the first generation system uses a handheld device to operate the capsule, the second generation system uses a robot-assisted operation, and the third generation uses a robot to fully automate the control of the capsule
    .

So, how safe and effective is the FAMCE system in clinical practice? A prospective study on the FAMCE examination of the stomach and small intestine carried out by the team of Professor Xie Xia from the Gastroenterology Department of Chongqing Xinqiao Hospital explored this problem
    .

Related research results were published in The Lancet Gastroenterology & Hepatology
    .

The results of the study show that the effect of FAMCE in completing gastric examination and lesion detection is similar to that of traditional gastroscopy
    .

In addition, FAMCE can also perform a complete small bowel examination
    .

Therefore, FAMCE can be used as an effective method for gastrointestinal examination
    .

The study was conducted in two centers.
Patients in the group were from two hospitals in Chongqing, aged 18-80, with suspicious stomach problems and no history of surgery
    .

The patient was screened for gastric lesions by FAMCE first, and routine oral gastroscopy was performed 2 hours later, and the results of the two examinations were compared
    .

The primary endpoint of the study was the complete detection rate of FAMCE on the anatomical structures of the stomach (cardia, fundus, body of stomach, angle of stomach, antrum and pylorus)
    .

The secondary study endpoint is the time required for FAMCE to complete the test and pass through the stomach, the rate of FAMCE compared to traditional oral gastroscopy for gastric lesions, and the rate of FAMCE's whole small bowel examination (because the small intestine is very long and tortuous, conventional Gastroscope and colonoscopy cannot be reached)
    .

In addition, the study evaluated adverse events
    .

From May 12 to August 17, 2020, 114 patients (average age 44 years; 63[55%] women) were enrolled in the study to perform imaging examinations of their stomachs and small intestines
    .

The results of the study found that the complete detection rate of FAMCE on the anatomical structure of the stomach was 100%
    .

The consistency between FAMCE and traditional oral gastroscopy is 99.
    61%
    .

The average completion time of FAMCE gastroscopy was 19.
17 minutes, and the average time of routine oral gastroscopy was 5.
    21 minutes
    .

In the gastroscopy of 114 patients included, FAMCE and conventional oral gastroscopy found a total of 214 lesions
    .

Among them, 193 lesions were detected in two ways
    .

Routine oral gastroscopy had 16 pathological findings that were not detected (12 cases of gastritis, 1 case of gastric polyp, 1 case of fundus xanthoma and 2 cases of gastric antrum erosion), and only 5 cases of FAMCE were not detected (4 cases) Gastritis and 1 case of gastric polyp)
    .

FAMCE can provide a complete small bowel examination for the enrolled patients, and intestinal lesions were detected in 50 (44%) patients
    .

In addition, two patients (2%) experienced adverse events during the study
    .

There is no evidence that capsule retention or serious adverse events related to FAMCE occurred
    .

Professor Xie Xia and her team stated in the paper that FAMCE will become another multifunctional and automated tool in the field of gastroscopy
    .

Image source: In a review article published by 123RF in the same journal of "The Lancet-Gastroenterology and Hepatology", Professor Jean-Francois Rey of the French Arnault Tzanck Institute said: "Robot operation and fully automated applications have opened up capsule endoscopy A new era of mirrors"
    .

He also added that “because FAMCE does not require manual operation, it should be able to minimize the risk of human error
    .

” He suggested that large-scale multi-center trials should be carried out in the next step to encourage the use of this technology in regions outside of China
    .

Because this technology has developed well in China and has shown promising results, relevant research results should be tested in other regions as soon as possible
    .

As a result, we see that the magnetically controlled capsule endoscope can be used for gastrointestinal examinations, providing a beneficial supplement to traditional gastroscopy
    .

It is expected that with the gradual accumulation of research evidence and clinical application and practical experience, the new magnetic control capsule endoscopy technology will bring more options for the early detection and diagnosis of gastric diseases
    .
